Just go to that window in the video and talk to the guy be patient wait tru the whole thing then when hes talking the 1st time talk to him again and wait through the whole thing and then he'll give you the flamethrower. No need to press rapidly, it wont give you nothing.

It's not after you get to the cathedral. It must be something else.
